Abstract: Introduction: Galactosemia is an inborn error of galactose metabolism, caused by the deficiency of some of the enzymes GALT, GALK1, GALE and GALM. In this study, six cases with galactosemia and their clinical, biochemical and molecular characterization are presented. Material and methods: The clinical data of six patients with galactosemia are presented, as well as the nutritional treatment and follow-up are described. The diagnosis was made with an expanded neonatal screen. In patients with a total galactose measurement > 10 mg / dL, the Beutler test was performed. Molecular analysis for the GALT, GALE and GALK genes was performed by Sanger sequencing. Results: Three patients presented clinical data at the time of diagnosis, two with acute neonatal symptoms, the third with chronic symptomatology, reaching diagnosis at school age. The other three were asymptomatic. All six patients were offered nutritional treatment based on the elimination of galactose. In four patients, biallelic pathogenic variants were identified in GALK1 and one in GALT. In the sixth patient, a homozygous intronic deletion was detected in GALK1, classified as SNP. Conclusions: Galactosemia has a variable clinical spectrum, therefore, its diagnosis requires both its detection by metabolic screening and the identification of a compatible clinical picture. As part of the approach, the genetic aspect is essential in order to offer individualized genetic counseling.
Resumen: Introducción: La galactosemia es un error innato del metabolismo de la galactosa, causado por la deficiencia de alguna de las enzimas GALT, GALK1, GALE y GALM. En este estudio se presentan seis casos con galactosemia y su caracterización clínica, bioquímica y molecular. Material y métodos: Se presentan los datos clínicos de seis pacientes con galactosemia y se describe el tratamiento nutricional y seguimiento. El diagnóstico se realizó con tamiz neonatal ampliado. A los pacientes con una medición de galactosa total > 10 mg/dL, se les realizó la prueba de Beutler. El análisis molecular para los genes GALT, GALE y GALK por secuenciación Sanger. Resultados: Tres pacientes presentaron datos clínicos al momento de su diagnóstico, dos con cuadro agudo neonatal, el tercero, con datos crónicos, pero el diagnóstico fue en edad escolar. Los otros tres pacientes estaban asintomáticos. A todos se ofreció tratamiento nutricional a base de eliminación de galactosa. En cuatro pacientes se identificaron variantes patogénicas bialélicas en GALK1 y uno en GALT. En el sexto paciente se detectó una deleción intrónica en homocigosis en GALK1, clasificada como SNP. Conclusiones: La galactosemia tiene un espectro clínico variable, por lo cual para su diagnóstico se requiere tanto de su detección por tamiz metabólico como por la identificación de un cuadro clínico compatible. Como parte del abordaje, el aspecto genético es básico a fin de ofrecer asesoramiento genético individualizado.
